What to expect

Being well-prepared for your appointment will ensure that Dr. Lewis has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. If you have any x-rays or MRI scans, please bring them with you. Your first visit may take a little more time than usual. There are important forms that need to be filled out so we may get to know you and your condition better. Also, there are forms to fill out if you wish us to bill your insurance or if this is an auto accident. We look forward to your first visit.
